On my own free, by my will and in my interest, I hereby make the decision and consent on granting
any personnel data to be processed by the operator of personal data: FSUE “TsAGI” Primary State
Registration Number (INN) 5013009056, Russia, 140180, Moscow Region Zhukovsky, Zhukovsky
Str., b.1. The personnel data processing is intended for: organizing the participation in International
Conference on Hi-Speed Vehicle Science & Technology, Moscow, 26–29 November, 2018. List of
personal data which on the personal data subject grants the consent to be processed: Application,
first name, family name, passport details, company, job, position, company headquarter, company
address, e-mail, telephone, accommodation address, permanent address, home address, city where
the participant will obtain visa in the RF. List of operations with personal data, which on the consent
is granted, the general description of operator's actions to be used for data processing: the processing
of the abovementioned personal data will be carried out by integrated personal data processing
(collection, systematization, accumulation, storage, clarification/updating/use modification,
distribution (inclusive the transboundary transfer), depersonalization, blocking and destruction of
any personal data).– The Federal Law # 152 by 27.07.2006 gives the general description of the
abovementioned data treatments. The timeframe within which the personal data subject consent is
valid as well as the way to revoke it, if otherwise is not stipulated by the Federal Law: the subject
personal data are due to be stored within period that is stated by the legislation of the RF. The
personal data are to be destroyed: upon reaching the personal data processing purposes, upon
liquidation or reorganization of FSUE “TsAGI” based on written personal data subject's appeal with
demand to stop his personal data processing (the operator will stop the processing of such personal
data within 3 (three) working days; the written notification will be sent to the personal data subject
within 10 (ten) working days. The subject has right via written request to get information that covers
his personal data processing (in conformity with § 4 art. 14 FL № 152 by 27.07.2006).
